Event Description

Join us for a celebration of the great things that
Connected Transitions has been doing. Connected Transitions is the Jackson Care Transitions Community - one of nine communities across Tennessee to improve patient care, reduce avoidable hospital readmissions and lower healthcare costs.

Community members will share stories, successes and new resources available to improve the health and wellness of Tennessee's seniors and reduce hospital readmissions.
