Accepting applications for enrollment of children for 2011-2012 program year. If child will be four years old on or before September 30 and you are interested in enrolling him/her in the Head Start program, you may complete application during registration day. The following documentation is needed to complete your child’s application: 1) Proof of child’s age 2) Social Security number for each family member 3)Name, address and telephone number of at least three emergency contact persons 4)TennCare and/ or insurance card 5) Proof of income
