Humboldt Head Start Recruitment
March 19, 2012-March 20, 2012 Humboldt Head Start will be taking applications for the 2012-2013 school year. If your child will be 3 or 4 on or before September 30, 2012, you may complete an application. The following items will be needed in order to complete an application: –
Child‘s Proof of Birth (Birth Certificate, or the Mother‘s Copy) –
Child‘s Social Security Card –
Child‘s Health Insurance Card –
Child‘s Immunization Record –
Proof of Family Income (Check Stubs, TANF/Families First letter or print out, W-2) –
Names, Birthdays, & Social Security Numbers of all household members –
3 Emergency Contacts (Name, Address, and Phone Number)
