Date / Time: January 8, 9, 10, 2012 8:30 a.m.- 5:00 p.m. Location: Chester County Head Start Center, White Ave., Henderson Items you MUST bring: Child’s name and birthdate, copy of TennCare card and Social Security Card for all members of the family. You must also bring child’s immunization records and income verified by W-2 form, income tax form 1040, and pay stubs accepted.
