Dates / Times: March 13, 2013 ………… 7:30 a.m. – 3:30 p.m. March 14, 2013 ……………………………………….7:30 a.m. – 5:30 p.m. Documents Needed: Proof of child’s age / birth certificate Social Security Numbers for each family member Name, address and telephone number of at least 3 emergency contact persons TennCare and / or insurance card Immunization Records from Health Dept. Proof of income (at least 3 check stubs)
