State Releases Report on Fatal Incident at Elemental Holdings

Elemental Holdings in Jackson has been ordered by the state of Tennessee to pay thousands of dollars in fines after an inspection following the death of a maintenance worker. Dan Baxter was killed May 31 while working at Elemental Holdings. According to his family, plant management told them an equipment malfunction may have been to blame for his death, which they said happened when a roll of aluminum fell and pinned him against a wall. In a report released Monday by TOSHA, inspectors found four serious violations, including improper us of a forklift. Three non-serious violations were also noted. In all, Elemental Holdings was fined $15,800.
