Registration for 2013-2014 Prek/Head Start program for those with children four years old on or before September 30. Documentation needed to complete application: 1)Proof of child’s age 2) Social Security numbers for each family member 3) Name, address, telephone number of at least 3 emergency contact persons 4) tenncare and/or insurance card 5) Proof of income
