Recruitment for income eligible pregnant women, and children birth up to the age of thirty-six months including children with special needs. Items to bring: TennCare card and any other insurance cards for the applicant Birth Certificate (Can be official copy or mother’s copy) Proof of Income: (W-2, income tax form, child support, or public assistance print out-SSI or Families First, or Social Security letter, at least 4 consecutive check stubs) Social Security card Name, address, and telephone numbers of at least three emergency contact persons.
