Accepting Applications
Humboldt Head Start will be accepting applications on Thursday April 25, 2013 from 8:00a.m. to 4:00p.m. In order for your child’s application to be completed, you must provide the following. 1. Proof of child’s age 2. Social Security number for each family member 3. Name, address, and telephone number of at least three emergency contact persons 4. Tenn Care and /or Insurance card 5. Proof of income Children with Disabilities will be given priority for enrollment including those with severe disabilities. Bus transportation is provided at a limited number of centers.
