Recruitment for income eligible pregnant women, children birth to the age of thirty-six months including children with special needs. Items to bring: Insurance cards for the applicant Birth Certificate (Can be official copy or mother’s copy) Proof of Income (W-2,at least 4 recent check stubs, income tax form, child support or public assistance print out-SSI or Families First or Social Security letter) Social Security card Name, address and phone numbers of at least three emergency contact persons.
